Registration
You will be required to be at the school two times during the registration period. Each day has specific activities. In order to attend ACEC you must meet the following criteria: Ø Score at or above the sixth grade level in both reading and math Ø Earned 10 or more high school credits Ø Live in Chicago Ø Be between 17 and 20 years of age. All other students may take the exam but will be placed on a waiting list. DAY 1: Testing: Prospective students must:
________________________________________________________________________________________________ DAY 2: Eligibility: There are a number of forms we need to maintain for students in order to qualify for our program. This eligibility folder must be completed before any student can start classes. From last attended high school: Official or Unofficial transcript of all high school credits Health Record from prior school. Shots must be current. Official release from prior school once accepted. From home: Proof of Address (Driver's license, state ID, current medical card, gas, light, phone bill) Proof of age (Birth certificate, driver's license, state ID Social Security Number (Social sec. Card, Driver's license or state ID if number on card, SS print out ) Proof of income from head of household (Current: check stub, med. card if receive money or stamps, social security, disability, unemployment, retirement) From Parent or Guardian: Completed YCCS Lottery Form (we will give you this form) Completed Lunch Form (we will give you this form) |
